The REACT service provides rapid assessment of patients identified as having urgent and unscheduled care needs, who could be safely managed in the community. This role is the key component in providing a single point of access for patients into a range of primary, secondary and social care services.

The service supports the Trust’s Unscheduled Care Agenda, providing an alternative pathway to manage patients in their own homes in order to both reduce unnecessary hospital admissions and or facilitate early safe discharge. An experienced Clinical Specialist [Physiotherapist/ Occupational Therapist / Nurse] with advanced assessment skills you will work predominantly within the Urgent Care Centre at the Royal Lancaster Infirmary alongside Urgent Care GPs identifying those who could be cared for at home and referring them into appropriate Community Health and Social Care Services – You will also act as assessor for the Rapid Response services receiving referrals directly from GPs or other health and social care professionals working in the community.

This represents an exciting opportunity to be part of a multi-professional service working in collaboration with both Primary and Secondary Care Providers throughout North Lancs and South Cumbria. Therefore knowledge of the newly formed Morecambe Bay CCG footprint would be advantageous. Independently managing your own caseload, you will be responsible for interpreting and analysing clinical and non-clinical facts to form an accurate diagnosis, discuss treatment options and agree a management plan with patients. You will also work with a variety of health & social care stakeholders to ensure timely assessment and delivery of a co-ordinated multi-disciplinary / multi-agency service. The service will provide patients with an alternative care option interfacing with primary and secondary care. Therefore it requires a proactive individual with excellent clinical knowledge, leadership and communication skills.

Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

Consideration will be given to those who do not hold a first degree providing that you have evidence to support recent academic development at Level 6 or above and /or equivalent experiential learning
