Job overview

This is a full-time post; however part time or job share would be considered. 

The role of the Cancer Support Worker will involve supporting and co-ordinating the pathway for patients living with and beyond a diagnosis of cancer.  The role will also include providing administrative and coordination support to patients, families, consultants, cancer nurse specialists and other staff involved in the cancer pathway.

You will work closely with the clinical team and will be a point of contact for patients and carers, coordinating all aspects of their aftercare including mammographic surveillance; organising Health & Wellbeing workshops; co-ordinating Holistic Needs Assessments; signposting patients to support services and resources; triaging patient phone calls; implementing policies and procedures and managing data. You will also be supporting the educational clinics for patients entering this pathway.

 

 

 

Main duties of the job

supporting and co-ordinating the pathway for patients living with and beyond diagnosis of cancer

providing administrative and co-ordination of support for these patients

working with consultants, cancer nurse specialists patients and families to ensure patients have access to services or advice should they have any concerns ahead of regular follow up

updating hospital systems and requesting of surveillance imaging

being point of contact for patients on pathway

Arranging and booking access appointments for patients that require treatment. As well as arranging education sessions for patients introduced on the pathway

completing holistic needs assessments for patients and providing necessary literature and advice on self managed care after a cancer diagnosis and treatment. 

Working for our organisation

We operate from three main hospitals - 
Furness General Hospital (FGH) in Barrow, the 
Royal Lancaster Infirmary (RLI), and   Westmorland General Hospital (WGH) in Kendal, as well as a number of 
community  healthcare premises including 
Millom Hospital and GP Practice,                               Queen Victoria Hospital in Morecambe, and 
Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency  Departments, Critical/Coronary Care units and various Consultant-led services. 

WGH provides a range of General Hospital services, together with an 
Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including 
outpatients,  diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities 
across Morecambe Bay.

Education and Qualifications

Essential criteria
  • GCSE English and Mathematics (grade A-C) or equivalent
  • NVQ 4 or 3 plus additional training to diploma level equivalent

Experience

Essential criteria
  • Relevant Health and Social care experience at Band 3 or equivalent
  • Communication with patients
  • experience with using internal hospital computer systems
  • experienced administration experience including use of Microsoft packages

skills, ability and knowledge

  • understanding and ability to deliver patient centred care
  • high level verbal and written communication skills
  • able to work independently or part of a wider team
  • Able to manage and prioritise workloads to meet the needs of the service and to meet changing deadlines
