Job overview

The Community Care Group has an exciting opportunity to recruit to a permanent band 5 position for 37.5 hours. This job role will initially be to work within the Alfred Barrow District Nursing Team for a period of 12 months. Following an initial 12 months, the location of this job will then be reviewed and the successful candidate may be moved to another district nursing team.

We would like an enthusiastic and highly motivated band 5 Community Nurse who will be happy to work in an evolving district nursing service. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.

The post holder will work as a registered Nurse within Community Services,

 accountable for providing evidence based, holistic nursing care of patients.

 

 

 

Main duties of the job

As a registered Nurse you will have direct and indirect contact with patients including those with long term conditions, their family and carers at each contact or as the need arises to deliver nursing care/education.

 

You will have social care responsibility to work with and refer to other care providers such as Acute Trust /GP’s /adult /mental health/statutory and out of hours services to ensure needs of the individual are met appropriately and safely.

 

The post holder will be required to work as part of the integrated team across community settings such as patients’ homes, residential and nursing homes. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.

Working for our organisation

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre. 

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. 

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. 

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay
